NEW APPROACHES TO THE TREATMENT OF CHRONIC CALCULOUS CHOLECYSTITIS
Chronic cholecystitis is one of important problems of modern medicine, so the possibility to apply therapeutic fasting in treatment of the disease is of great interest. The purpose of our work was to study the influence of a dietetic fasting therapy on the course of chronic calculous cholecystitis. There were developed the criteria for the choice of patients with chronic calculeus cholecystitis with the account of acoustic properties and total volume of concrements. The criterion for the patients choice will allow to prognose the results of therapeutic fasting and to improve in this way the efficiency of management of this widely spread disease. Therapeutic fasting being the efficient, economically advantageous, atraumatic method can be used successfully in treatment of patients with both non-complicated calculous cholecystitis and latent form of disease preventing the development of clinic manifestations of calculous cholecystitis and its complications.

THE INFLUENCE OF A LONG HUNGER CURE ON THE MORPHOFUNCTIONAL STATE OF CENTRAL AND PERYPHERIAL ORGANS OF IMMUNOGENESIS
Therapeutic fasting (dosed complete hunger) is getting ever broader use as a simple, available and efficient means of treatment of various chronic diseases. We have distinguished the following adaptation periods of the immuncompetent organs to full long hunger: a period of initial changes: 1st day; a period of expressed immune-morphological rebuilding: from the 3d day of full hunger; a period of resistancy of adaptive syndrome: 7 days of full hunger; a period of restitution of limph organs: 7 days of a rehabilitation period.
We found that reaction of immunogenesis organs in a long full hunger dynamics has a regular system and phase character that being acompanied by a sharp decrease of the immunecompetent cells.
Today it is generally recognized that long hunger results in an increase of a non-specific resistancy of an organism and it is of a definite sanogenous effect. Full hunger is accompanied by a decrease of a humoral and to a greater extent, cellular immunity with the simultaneous increase of a non-specific immune protection due to increased quantity and functional activity of the microphages and natural killers in thimus, spleen and limph nods.

STRUCTURAL REBUILDING OF LIVER IN CONDITIONS OF LONG FULL HUNGER TREATMENT
In conditions of histological studies of rats' liver in a process of hunger, in conditions of a free animals access to water the heterogenic changes of hepatocytes' structure was revealed its level and form depending on the term of a nutrition deprivation.
The most important feature of hepatocytes of animals in hunger was a sharp increase of the autophagia process.
Under the full long hunger in a liver distrophic processes are dominating compared to regenerative in the first three days of hunger in a form of a grainy and fatty distrophy of hepatocytes. After the third day of hunger the improvement of liver's morphology and activation of regeneration processes was. observed. A manifesting feature of the structural reconstruction of hepatocytes in full hunger is a sharp increase of lysosomes and autophagosoms with the simultaneous reduction of chanals of endoplasmatic net and mitochondria.

MORPHO-FUNCTIONAL STATE OF TROMBOCITES IN PATIENTS WITH THE ISCHEMIC HEART DISEASE IN A PROCESS OF THERAPEUTIC FASTING COMBINED WITH HIRUDOTHERAPY
Numerous works proved that the platelets play an important role in pathogenesis of ishemic heart disease and its complications participating in the .progress of atherosclerosis, elevated coagulation blood potential, spasm of coronary arteries and disorders of microcirculation.
Our electron-microscopic investigations of blood platelets in patients with ishemic heart disease showed drastic changes in a structure and function of blood plates.
The application of therapeutic fasting diet and hirudotherapy in patients with ishemic heart disease had positive effect on a microscopic organization of platelets.
In a renewal period the majority of platelets was in norm as to the structure but distinguished itself in clearness of membrane components of cytoplasm elements and large quantity of a glycogen granules.
On discharge from hospital the ultrastructural elements of a cytoplasm in patients with ishemic heart disease by the end of a renewal period of therapeutic fasting diet were conserved better compared to the platelets of the patients who got medicamental management.
The electronic-microscopic studies of blood trombocytes in patients with chronic heart disease showed great changes in a structure and function of blood platelets in a process of therapeutic fasting diet and hirudotherapy.

THE APPLICATION OF DIETETIC FASTING THERAPY IN TREATMENT OF PATIENTS WITH CHRONIC NON-SPECIFIC LUNG DISEASES
The chronic non-specific lung disease management is one of the urgent problems of a nowadays pulmonology. Of special importance in this respect is a therapeutic fasting. The aim of the present work was to study the efficiency of a therapeutic fasting in patients with chronic obstructive lungs diseases.
In a therapeutic fasting process the improvement or normalization of indices of external respiration was observed.
The therapeutic fasting is a high efficient non-medicamental treatment method of a chronic obstructive bronchitis and bronchial asthma which is well tolerated by patients, it has no complications and side-effects. It ensures stable remission both on the part of basic disease and accompanying diseases. It is recommended for use in therapeutic practice upon a correspondent training of a personnel.

THE DYNAMICS OF A BIOTIN EXCHANGE DURING THE FASTING DIETETIC THERAPY
IN PATIENTS WITH THE DIGESTIVE ORGANS DISEASES
All pathological processes are accompanied as a rule by the changes in a vitamin exchange of an organism.
The purpose of this work was to study the possibility of a compensation of biotin insufficiency at the expense of assimilation of a biotin depot by an organism.
An organism in extreme situations begins to assimilate its own biotin reserves and mobilizes it to support the important biological processes. With the beginning of a recurrent income of a biotin with the foodstuff there takes place a cessation of the use of vitamine storage, its microbic synthesis is being renewed, this can explain the increase of a biotin content in a dayly urine.
Based on the above stated a conclusion can be made that in case of a lack of income of a biotin from outside the biotin supplement of an organism is carried out through the endogenic reserves.

THERAPEUTIC FASTING AND DIETETIC THERAPY IN GERMANY
Therapeutic fasting and nourishment in a clinic for rehabilitation/social medicine.
The rehabilitation of workers and farmers suffering of the metabolic syndrome worked most effectively by the combination of fasting, movement, nourishment correction and health education - adapted to the specific skill level of the patients.
Fasting time of 14 to 28 days made quick reduction of all bodily risk-factors as well as the inner acceptance of disclaimation and even for the times at home; so it succeeds to accomplish a consistent nourishment therapy. Necessities: therapy duration of 4 till 6 weeks, adult education: presentation from doctors and experts, group discussions, "kitchen college" and psychotherapeutic support.
Excellent long-term results even over a period of two years were documented in a controlled scientific study.
Fasting for healthy people: adult education. A short period fasting of 5 to 10 days shows that a human being is able to live without food and will be fully capable and clear in mind. During that time key interrelations of live are experienced, resulting finally in self-finding and self-development.

TREATMENT WITH NATURAL PECTINE PASTES OF PATIENTS WITH CHRONIC ENTERITIS AND THE POSSIBILITY OF PECTINE-CONTAINING PASTES APPLICATION IN A REHABILITATION PERIOD OF A FASTING DIETETIC THERAPY
Recently much attention is paid to the study of the effect of food fibers in treatment of chronic diseases of intestines.
One of the directions of this work is the study of the curative action of pectine-containing apple paste on clinical-laboratory indices in patients with chronic enteritis.
The use of an apple pectin paste in a complex management of patients with chronic enteritis inhibits the initially increased tonic violations of an intestine thus having a regulating effect on the disordered motoric function of the intestine.
The investigations showed that the absorption processes in a small intestine are stimulated under the influence of a course intake of pectin-containing apple pastes. Complex treatment with the use of pectin-containing apple pastes in patients with chronic enteritis affects mostly the main pathogenetic chains of a disease. This allows to avoid a polypragmasia, complications due to medicines, allergic reactions and it can be widely used in medical practice in treatment patients with intestine pathology.

THE EFFICIENCY OF FASTING DIETETIC THERAPY COMBINED WITH THE INTAKE OF MINERAL WATER "POLYANA KVASOVA" IN PATIENTS
WITH THE INFECTION-DEPENDENT BRONCHIAL ASTHMA
The aim of the work is to study the efficiency of a treatment combination: fasting dietetic therapy and moderately mineralized hydrocarbonic mineral water "Polyana Kvasova" on the background of basis generally excepted therapy in patients with infection-dependent bronchial asthma of a light and medium degree.
It was proved that the therapeutic fasting and the use of a hydrocarbonic mineral water "Polyana Kvasova" in patients with infection-dependent bronchial asthma on the background of basis generally excepted therapy favors the improvement of a general state of a patient, bronchial passability, favors prolonged remission of a disease. It can be recommended for the treatment of patients with the infection-dependent bronchial asthma of a light and medium degree in ambulator-policlinic conditions.

DIETETIC FASTING THERAPY IN PATIENTS WITH COMBINED PATHOLOGY OF THE DIGESTIVE ORGANS WITH ALLERGIC COMPLICATIONS
During 35 years by the method of dietetic fasting therapy (DFT) over 2000 patients had been managed, among them 757 with combined disorder of the digestive organs with two or more diseases of a gastroduodenocholepancreatogenic zone or hepatobiliar pathology and inflammatory or functional disorders of various parts of the intestines.
The task of the study was to examine vitamin supply of patients with combined pathology of digestive organs with allergic complications before the treatment and in different periods of fasting and rehabilitation before and after DFT.
For this purpose 14 fat- and water-soluble vitamins of a blood serum in 255 patients was studied.
The examination of vitamin in all periods of DFT showed that DFT resulted in further decrease of a vitamin supply of patients' organism.
Taking account of high frequency of allergic complications in gastroenterologic patients we were interested in a study of the state of hydrolysis of oligopeptides as a main source of antigens and changes in a local immune system of mocous membrane of small intestine in patients with pathology of digestive organs complicated with allergy under the action of treatment with DFT.
Clinically at the end of the course from 71% to 100% of patients recovered (dependent on the allergologic diagnosis of patients with combined pathology of the digestive organs). Positive dynamics of clinical symptoms was present in all patients.
Therapeutic action of DFT was realized predominantly via complex compansatory-adaptive sanogcnic mechanismds of influence at all levels of supplement of a normal homeostasis of an organism particularly in patients with combined patrhology of digestive organs the DFT favoured the processes of readaptation in food assimilation preventing thus antigenic agression in the meantime stimulated protective-barrier properties of a local immunity of mocous membranes.

LOCAL IMMUNITY STATE OF MUCOUS MEMBRANES AND GENERAL IMMUNITY INDICES IN TREATMENT OF ALLERGIC COMPLICATIONS OF COMBINED PATHOLOGY OF DIGESTIVE ORGANS BY THE FASTING DIETETIC THERAPY METHOD
The patients with combined pathology of the digestive organs with allergic complications are characterized by:
o violation of protective-barrier function of a local immune system of mucous membranes on the background of disordered digestion processes;
o activation of B-system of immunity with the development of different types of a hypersensitivity;
o recurrent T-cellular immunedeficiency;
o decrease of an inborn non-specific resistance of an organism.
The therapeutic action of a fasting diet is realized in patients with combined pathology of digestive organs with allergic complications first of all through the increase of a protective-barrier function of a local immune system of mucous membranes (VALT) and a series of adaptive-compensatory, sanogenic reactions in the limits of T- and B-chains of general immunity and inborn non-specific resistance of an organism, directed to normalization of the immune status of patients. Thus dietetic fasting therapy is a method of choice in case of allergic complicated pathologies of the digestive organs.

THE IMPORTANCE OF FAST FOR HEALTH
The majority of people are eating in order to be full regardless of the amount and the quality of food. The consequence is that those taking monotonous, low-valued, hardly assimilated meals suffer from digestive diseases and those overeating are also subject to different kinds of sickness.
People working not enough physical and overeating mostly animal and protein food (meat, eggs, fish) are poisoning themselves with the dissipation products of a protein substance that is stored in an organism and they grow old more quickly. Very strict and long fasting is unhealthy. Dietary food in order to be useful and not hurt organism should correspond to requirements of an organism and the volume of the work being done.
During the Lent one should consume beans, pees, lentil, potatoes, onion, carrots, vegetable oil, sore cabbage, dried fruits, brown bread, milk and eggs (except the first and the last weeks). During the Lent one mustn't consume much salt and alcohol. The apples are to be eaten uncooked (some 5-6 apples a day). Such a fast will be redemption for both the soul and body.

SANATIVE THERAPY FOR PEOPLE LIVING IN CONDITIONS OF POLLUTED ENVIRONMENT
Scheme A. Treatment course oriented to desintoxication of an organism and strengthening of defensive properties, including the immunological status, and normalization of metabolism; consists of three stages.
Stage 1 (a three-day fasting period according to therapeutic fasting (TF)).
During this period the patients do not take food, they drink 1.2 - 1.51 of aqua distillata: in dependence of the body weight. On the day of hospitalization or at home on the first day the patient takes an evacuant (1 table spoon of magnesium sulfate per one glass of water), and in the evening he purges the intestines with the evacuant enema. Next day in the morning the evacuant enema is repeated. During the daytime the patient only drinks distillated water. The same is during the third day. This period obligatory includes:
o daily three times cleaning teeth and the mouth cavity;
o walk in the fresh air during some 3-4 hours;
o everyday shower, underwater shower-massage or swimming in the water-pool;
o massage of the spine and the joints.
Stage 2 (a two-day, of a dietary recovery).
During the first day the patient in dependence of the sugar level in blood and acidity drinks juices of: grapes, birch, carrots or apple. It is recommended 11 of juice deluted with water in correlation 1:1 per day.
During the second day the patient has for breakfast 2 creamed apples or 2 carrots and 100 ml of kefir. 3 hours later the patient should eat 2 potatoes baked in their jackets and drink 200 ml of kefir. For dinner he has creamed vegetables, fruit, 200 ml of undiluted juice. Three hours after dinner - once more 2 potatoes baked in their jackets and 200 ml of kefir. For supper he has soup of the oat-flakes or buckwheat. Salt is not used in making food. Compulsory procedures of the stage 1 should be carried out.
Stage 3 (a fefteen-day course of phytotherapy).
Three times a day 30 minutes before meals drink 100 ml of the phytotea "Karpaty". It is better to make tea on melt structured water or aqua distillata (pH>7). When taking food it is recommended to take 50-90g of a sorbent "Fitocrisps". Every day raw fruit and vegetables, 250-300g of juice should be taken.
In special cases the patients are prescribed the acupuncture, psychotherapy, massage, water procedures.
Broth, sugar, meat are excluded from the diet.
The method of TF is contra-indicated for patients with cancer, leucosis, tuberculosis, diabetes of medium and heavy stages, liver cirrhosis of ascitic stage, cachexy of any origin and in cases of acute disease.
Scheme B. When the patients refuse from a three-day fasting or there are contr-indications to it, or in case of hyperacid inflammatory states of a gastroduodenal zone the first and second stages are replaced by a 5-7 day intake of "Fitosorb" enterosorbents or "Tseofitosorb" on the background of the fruit anf vegetable rich diet, excluding the high-melting fats, meat, salt with predominant uncooked and stewed food. Physical activity and balneological procedures are necessary. Further stage of management is carried out according to the stage 3 of the scheme A. The phytotea "Karpaty" can be replaced with the alcoholic tincture "Ehinofit" during 10-15 days; 20 drops 3 times per day.
In conditions of ecological pollution, including the radionuclides contamination it is recommended to take the health sanative courses once per every 3-4 months. The courses will ensure regular elimination of radiotoxins from an organism, improve immune tolerance, metabolism and antioxidant safety of an organism.
RESOLUTION OF THE INTERNATIONAL SCIENTIFIC-PRACTICAL CONFERENCE "HEALTHY NATURE FOOD. DIETETIC FASTING THERAPY"
(October 7-8, 2000, sanatorium "Polyana", Svalyava district, Transcarpathian region)
70 participants took part in the conference: 17 Doctors of Science, 15 Candidates of Science, the doctors - practitioners from Poland, Hungary, Germany, Ukraine; also participated Doctor of Economics, professor Osyp Moroz (USA).
The reports were made by 35 members, 15 poster sessions were presented. 15 participants took part in discussion.
The conference stated that Ukrainian scientists are intensively working in the direction of the use of nutrition for the health strengthening and maintenance of the nation, decrease of a level of alimentary-dependent disorders with the aim of the prolongation of life, decrease of invalidation and mortality of the population.
The reports of the scientists from Ukraine, Germany and Poland on the scientific substantiation of the therapeutic fasting method, its efficiency in the recovery of various contingents of the population were summarized. At the conference the problem of the ecologically - defensive diet was discussed which is of a special urgency for Ukraine in a post-chernobyl period. It was recommended to work in close cooperation of the scientists and manufacturers in realization of the special-purpose foodstuffs, which can serve the basis for a wide-ranging prevention of diseases.
In the frameworks of the Decree of the President of Ukraine L. Kuchma on the preventive health-improving activity among the Ukrainian population the participants of the conference appeal to all Rectors of medical higher establishments to pay due attention to the necessity of teaching the Nutriciology and Phytotherapy in training physicians of a new generation - the XXI-century doctor. This is of particular importance in realization of the program of training a family doctor, increase of the level of preventive-therapeutic work and carrying out intensive educational work among all layers of population. The international experience proved natural nutrition schools to be the most progressive form of the educational work with the population. At the conference the lack of the concept of the XXI century nutrition taking account of the current economical and ecological state of Ukraine was stated.
The conference refers to the scientists who have their own achievements in the therapeutic diet and prevention of basic diseases with the appeal to introduce it into practice of health protection. We think that it is necessary to create a new, up-to-date dietary service in Ukraine.
We support the proposition of the foreign delegations (Polish, German) to edit the materials of the conference in English in order to get acquainted with the scientific works of Ukrainian scientists.
According to the proposition of the German delegation the second international conference will be held in Germany in 2001. The participants of the conference appeal to the authorities of Ukraine to create favorable conditions for the Ukrainian manufacturer in realization of the special-purpose foodstuffs (infants food, dietary products, biologically active supplements for sportsmen).
On the behalf of the conference special thanks are addressed to the chief organizer of the conference professor Bilas Bohdan -Director General of the German-Ukrainian academy.
